COVID-19 VACCINATION INFORMATION
Please see the following website for information on COVID 19 vaccinations in North Carolina.
As a precaution, to lower the spread of any upper respiratory illness including Flu and Corona Virus in our community, we ask that all of our patients who are showing any of the following symptoms, please call the office at 828-433-0225 during our office hours of 8:30am to 5pm Monday through Friday to be advised of treatment availability.

Fever, cough, shortness of breath, sore throat, body aches and headaches.

Our facility will be treating patients via telephone, patient portal and virtual visits if they are showing these type symptoms.

If you are experiencing emergent respiratory symptoms contact 911 or your local hospital for further instructions.  Please alert any medical personnel of you symptoms before they attend you.

Advanced Care Planning Classes

We now offer Advanced Care Planning Classes coordinated with Burke Hospice at no cost in our office.  A Living Will and Health Care Power of Attorney can be discussed, executed, witnessed and notarized at this class.

Make Each Doctor’s Visit Work For You!

Bring all of your medicines, including over the counter medicines, with you to your appointment.  Talk with your doctor about all the medicine you are taking, even those that were given to you by a different doctor.  Don’t stop taking any prescription medication without first talking with your doctor.

Bring a copy of your current Insurance Cards.

Bring in copies of all your Advance Directives such as Living Will, Health Care Power of Attorney, DNR or MOST forms.  If you do not have advanced directives ask your doctor about this during your appointment.

Write down your questions and concerns.  Ask questions.  Take notes.  Tell your doctor when you don’t understand.  Learn as much as you can about how to care for your illness.  The more you know the better for your health.

If you need help ask a caregiver, friend or family member to join you.

Please provide us with your medical history, as well as information about any care you obtain outside of our practice, to include your current medications including prescription and over the counter), recent test results, visits to other doctors and healthcare providers, hospitalizations, and emergency department visits.
